- Name of degree or course – Below the name of the university, you will find the name of the degree or course for which the transcript is requested.
- Name of the document – Transcript is known as a transcript of records, students mark statements, consolidated statements, etc. So don’t be surprised if the name of the document differs, the purpose will be the same.
Body
- Name of the student
- Date of birth
- College from where the student has completed graduation/post-graduation
- College code
- Year of passing
- Subject code and subject name
- Total marks obtained and year of passing for each subject will be mentioned beside the subject in a tabulated form

At the bottom of the document, you will find
- Signature of issuer
- Official seal of the issuing authority
